How to arrange Testing at AWTA Product Testing 1. Download and complete the Test Request. Refer to the AWTA Product Testing Fees list. (Note Aust. GST has not been applied) Ensure you include: Contact Person Email Address Company Name Company Address Daytime Phone Number The Official Name of the product or a Reference Any Sample description details you wish to appear on the test report The Sample's: composition and end-use, density, g/m2, etc Type of Testing required. Refer to Testing Fees List. If required, your Company's Official Order Number Ensure the form is authorised, signed and dated. Carefully check the "Terms" provided for testing and consulting services 2. Delivery Instructions Send or deliver your Test Request and Samples to: AWTA Product Testing Level 1, 191 Racecourse Road Flemington Victoria 3031 Australia 3. Payment Details If your company has a credit account with us your samples will be tested and the reports and invoice emailed to you immediately on completion. Two hard copies of the report with the tax invoice will be mailed to your company address to the attention of the designated contact. If your company does not have a credit account with us, a proforma Tax Invoice will be emailed to you when the sample is booked into our system. The proforma Tax Invoice provides several options on how you can make payment. Options include credit card, bank transfer and cheque. All payments must be in Australian Dollars. Please note: AWTA Product Testing WILL NOT provide details or reports until payment has been confirmed. On confirmation of payment, a test report will be emailed to your designated contact in PDF format and a hard copy of the report with the Tax Invoice will be mailed to your company address to the attention of the designated contact. If testing has been completed and payment has not been received a Tax Invoice will be emailed to the designated contact. On confirmation of payment by our financial institution all reports will be immediately provided.
